Registrar of Vital Statistics: Birth & Death Records
This Office is the repository for all birth and death records of anyone who was born or passed away in the Village of Sleepy Hollow, formerly known as
North Tarrytown.
*To obtain a Certified Birth or Death record:
Visit the Village Clerk's Office, located on the 2nd Floor, at 28 Beekman Ave.
Monday thru Friday between 10:00am to 3:00pm (wait time may be up to 20-30 minutes)
Each Birth Certificate or Death Certificate costs $10. (This fee is set by NYS.)
All Certificates/Records are certified and have a raised seal.
Certificates for the purpose of genealogy records costs $22. each.
If you would like to request a certified birth or death record by mail, follow these instructions:
- Send in a notarized letter with your request (or have the Birth Record application notarized)
- Send in a copy of your government-issued ID with the notarized letter or application.
- Send in a self-addressed and stamped envelope.
*For express/priority mail please use the US postal service. **We do not use FedEx nor UPS. - Send a money order or check for $10. made out to the Village of Sleepy Hollow
(each certificate is $10). - Mail to: Village Clerk, 28 Beekman Avenue, Sleepy Hollow, NY 10591
Who is eligible to get a copy of a birth certificate?
- The person named on the birth certificate.
- A parent of the person named on the birth certificate. The requesting parent's name must be on the birth certificate.
- A spouse, child or others, but only by order of a New York State court.
Who is eligible to get a copy of a death certificate?
- The spouse, parent, child or sibling of the deceased (MUST show/send proof of relationship)
- Other people who have a:
- documented lawful right or claim
- documented medical need
- New York State Court Order
If you are not the spouse, parent, child or sibling of the deceased you must document a lawful right or claim. For example, you may need a death certificate to claim a benefit. You would need an official letter from the agency saying you need the death record to process the claim.

Please call 914-631-0800 during non-business hours, to obtain a burial permit, if necessary. (4:30 pm to 8:30 am, weekends and holidays).
